What determines the head photo in the android app. It appears to just stay on random shows I’ve watched recently for ages. Can this be changed or it is a random pick? Where it says “welcome **** VIP since ****”

This is based on your selected profile image. To reset it go to https://trakt.tv/settings/advanced or to set it to something else go to any movie, show, season, or episode page and click “Set Profile Image” in the sidebar.
