Every so often a random episode of a show I marked off months to years ago pops up in my Up Next. At first I marked them off thinking I missed it (that messed up my watches) and then I stopped doing that and sometimes they’ll disappear but not always. Is there any way to fix this or is it just a thing that happens?

That might happen for a number of reasons, like, for example, someone adding bad data on TMDB and we removing that episode later after it was reported (so the episode disapears after we do that).
I would need an specific example of an episode where that happened so I can check exactly why it showed up again. This usually doesn’t happen randomly, there is probably a reason for each case.
Currently the episodes showing up in my Up Next that I previously marked are
Helluva Boss 2x13 Sinsmas
Suite Life on Deck 1x22 titled Episode 22
I’ve removed episode 1x22 of Suite Life on Deck, since it was bad data from TMDB (that episode shouldn’t exist).
As for Helluva Boss, that was because of a change on TMDB that now considers “Queen Bee” as episode 2x05, that pushed the other episodes 1 number ahead, so it created an extra episode. You can mark it on the same date you watched episode 2x12, as each episode after 2x05 was pushed one number ahead.
